The following questions cover the necessary checks we need to make to assess your suitability to volunteer.

Have you ever had any personal contact with Children’s Services/Social Services/Social Work Department or NSPCC in connection with children in your care? Have any of your children been subject to Child Protection, Child In Need Plan or Early Help Assessment (EHA)?*

If you do not declare existing or spent cautions or convictions you may not be selected. However, if you declare any of the above it may still be possible to become a volunteer.

The information I have provided is true and I know of no reason why I would be unsuitable to be a Home-Start volunteer.*

I give permission for Home-Start to carry out a criminal records check at enhanced level with the appropriate agency. I understand that my National Insurance number, passport and/or driver's licence may be required for identification purposes. I understand that I will need to show three documents for identification purposes, e.g.passport, driving licence, council tax bill.*

I understand that Home-Start may hold personal information about me on paper records and on database, including sensitive information such as age, race, sex and disabilities, that they will use for their monitoring purposes in accordance with GDPR legislation. I agree to them holding this information and understand that it may be shared anonymously with Home-Start UK for Quality Assurance purposes. I understand that I may ask to see my records at any time and I can view the Home-Start Privacy Policy on the website. These records will be retained for two years after I finish volunteering with the organisation and then they will be anonymised. (I can request to extend this retention period if I require a reference from Home-Start).*

I am happy for Home-Start Richmond, Kingston and Hounslow to contact me about future training, fundraising activities and other ways to get involved. I understand that I can discontinue these communications at any time by emailing info@homestart-rkh.org.uk.*

I understand that Home-Start asks for a minimum commitment of 2 hours per week (plus travel time) on a regular basis for at least one year.*
