In order for Centre 33 to support your young person we need to record details of you and of your family needs.

All information written and discussed is private and confidential within Centre 33. However, in exceptional circumstances if we believe someone is at serious risk or immediate harm, we would contact services that may help. We would always try and tell you first.

We would like your details as Young Carers Champion to be shared with young people, their families and local services that support Young Carers.  We would also like to list your name on our website as the named Young Carers Champion for your setting. This is so that young people can be aware of who their Young Carers Champion is.
